ABSTRACT

【Objective】 To investigate the independent risk factors of urinary tract infection (UTI) in recipients under-going renal transplantation of donation after brain death (DBD), so as to provide a theoretical basis for the prevention and control of postoperative UTI. 【Methods】 A retrospective study was conducted for recipients who received renal transplantation of DBD in our hospital during Jan.2021 and Dec.2021. The recipients were divided into the infection group (n=26) and non-infection group (n=74) according to the incidence of UTI 3 months after operation. The risk factors of UTI were identified with univariate and multivariate analyses. 【Results】 The incidence of UTI was 26%. Univariate analysis showed that gender, postoperative urinary fistula, time of indwelling catheter and time of indwelling double J tube were the influencing factors of UTI (P<0.05). Forward stepwise regression analysis showed time of indwelling double J tube (OR=1.086,95%CI:1.003-1.177,P=0.042) and time of indwelling catheter(OR=4.687,95%CI:2.064-10.645, P<0.010) were the independent risk factors of UTI (P<0.05). 【Conclusion】 The time of indwelling catheter and time of indwelling double J tube are the independent factors of UTI after renal transplantation of DBD.

ABSTRACT

Objective:To explore the accuracy of 18F-fluorodeoxyglucose (FDG) PET/MR in preoperative localization of refractory epilepsy patients with conventional MRI negative. Methods:From August 2016 to December 2018, 57 refractory epilepsy patients (36 males, 21 females, age (24.0±10.3) years) with conventional MRI negative who underwent surgery in Xuanwu Hospital were retrospectively enrolled. All patients received interictal 18F-FDG PET/MR before surgery and the epileptogenic foci were determined by using visual and semi-quantitative methods. Patients were followed up for 1 year and the surgical outcome was evaluated according to Engel classification. The sensitivity, specificity and accuracy of 18F-FDG PET/MR in locating epileptogenic foci were calculated according to surgical resection and followed-up results as the " gold standard" . Results:Of 57 patients, 51(89.5%, 51/57) showed single or multiple hypo-metabolism focus on 18F-FDG PET/MR, and 6(10.5%, 6/57) showed no abnormal metabolism changes. The microstructure abnormality was found in 18 patients (31.6%, 18/57) on 18F-FDG PET/MR images. Follow-up results were obtained from 46 patients, and 84.8%(39/46) with seizure improvement (Engel Ⅰ-Ⅲ). The sensitivity, specificity and accuracy of 18F-FDG PET/MR in preoperative localization of epileptic foci was 90.0%(27/30), 3/16 and 65.2%(30/46), respectively. Conclusion:18F-FDG PET/MR is helpful for the detection of epileptic foci in patients with MRI-negative refractory epilepsy, and can provide reliable information for further surgical treatment.

ABSTRACT

OBJECTIVE@#To observe the effect of cinobufagin on transient outward potassium current () in rat dorsal root ganglion cells of cancer-induced bone pain (CIBP) and explore the possible analgesic mechanism of cinobufagin.@*METHODS@#Whole cell patch clamp technique was used to examine the effect of cionbufagin on in acutely isolated dorsal root ganglion (DRG) cells from normal SD rats and rats with bone cancer pain.@*RESULTS@#The DRG cells from rats with CIBP showed obviously decreased current density, an activation curve shift to the right, and an inactivation curve shift to the left. Cinobufagin treatment significantly increased the current density and reversed the changes in the activation and inactivation curves in the DRG cells.@*CONCLUSIONS@# current is decreased in DRG neurons from rats with CIBP. Cinobufagin can regulate the activation and inactivation of current in the DRG cells, which may be related to its analgesic mechanism.

ABSTRACT

Epilepsy,a group of chronic neurological disorders characterized by spontaneous and recurrent seizures and learning and memory impairments, results in transient brain dysfunction due to sudden abnormal discharge of brain neurons. The pathogenesis of epilepsy is very complicated and has not yet been fully elucidated. The imbalance between excitatory glutamate and inhibitory gamma-aminobutyric acid (GABA) neurotransmitters in the central nervous system and changes in ionic functions of N-methyl-D-aspartate receptors directly induce epileptic seizures. The endocannabinoid system plays an important role in retrograde synaptic transmission and exerts the anti-epileptic effect in cannabinoid receptor 1 (CBR1) dependent manner by regulating the synaptic transmission of glutamatergic and GABAergic neurons and homeostatsis of ionic channel function.Elucidating the specific mechanism of action of CBR1 signaling pathway in epilepsy, can provide an effective theoretical basis and novel drug's target for clinical treatment of epilepsy.

ABSTRACT

Huntington's disease (HD),a single-gene autosomal dominant neurodegenerative disease,is pathologically characterized by the great loss of striatal neurons in the brain.Clinical manifestations of HD show involuntary dance-like movements, cognitive function and neuropsychiatric symptoms. The pathogenesis of HD is concerned with the protein expression of mutant huntingtin which leads to the selective degeneration of medium spiny neurons in the striatum and the onset of the disease.Cannabinoid receptor (CBR) plays a key role in the release of neurotransmitters, synaptic plasticity, gene expression and modulation of.neuronal activity through the CBR1 and CBR2 signaling pathways.Recent studies have found that CBR-mediated phosphoinositide3-kinases/protein kinase B/mammalian target of rapamycin complex1/brain derived neurotrophic factor, neuropeptide Y/neuronal nitric oxide synthase signaling pathway play a vital role in the regulation.of striatal neuroprotection in HD.This review reveals the research progress of CBR1 related signaling pathways in HD,which provides new theoretical basis and drug targets for the prevention and treatment of HD.

ABSTRACT

Objective To investigate mRNA expressions of bcl-2-associated athanogene1 (BAG-1), epithelial growth factor receptor(EGFR)in triple negative breast cancer(TNBC). Methods Polymerase chain reaction (PCR) method was used to detect mRNA expression levels of BAG-1 and EGFR in 51 TNBC cases combined with adjacent tissues in Taihe Hospital of Shiyan City from October 2013 to August 2014, and the relationship between gene expression and the clinicopathologic parameters of TNBC patients was analyzed. Results The relative expressions of BAG-1 mRNA and EGFR mRNA were 0.57±0.25,0.61±0.21 respectively in TNBC. The expression level of mRNA in breast cancer (0.19±0.12, t = 5.12) was higher than that in adjacent tissues (0.21±0.11, t = 7.17), and there was no significant difference (P< 0.001). The difference of expressions of BAG-1 mRNA and EGFR mRNA in TNBC patients with different clinical stage or lymph node metastasis were statistically significant (all P< 0.05), but there was no significant difference in mRNA expression level of the patients with different tumor diameter or age (all P > 0.05). Conclusion BAG-1 mRNA, EGFR mRNA are highly expressed in TNBC, which may be related with the poor prognosis and may be a molecular index for predicting the prognosis of TNBC patients.

ABSTRACT

Since the establishment of the People’s Republic of China, the development of China’s official de-velopment assistance for health has been going through three phases. To date, it has developed in many forms, inclu-ding the dispatch of medical team and the construction of health facilities. Since the market reforms, the global con-text together with the domestic socioeconomic foundations have changed; the impact of China’s relatively simple and segmented development assistance for health on the development of health systems in developing countries is limited;the effectiveness of assistance has been watered-down due to segmentation and vague management and accountability systems, as well as the lack of an overarching strategy;China’s health institutions, techniques and products suffer va-rious obstacles in their transfer to other countries where rules are dominated by western countries;compared with the increasing and multiple demands of development assistance for health from developing countries, the capacities of co-operation need to be further developed. As the paper suggests, use the “new major-country” and “new morality-in-terest” and“human oriented” concepts, as well as the ideology of“aid in order to develop, develop in order to coop-erate, so as to develop hand-in-hand” to guide China’s development assistance for health;innovate stereo-aid models to adapt to the changed foreign and domestic socioeconomic context; reform the development assistance for health management system and define rights and obligations appropriately;strengthen coordination and information sharing;link development assistance for health with global health governance to promote a maximized spillover effect;mobilize the civil society with strengthened guidance and supervision.

ABSTRACT

A novel method based on cross sensitivity of cataluminescence (CTL) generated on the surface of a nanometer composite oxide was proposed for simultaneous determination of benzene and trimethylamine (TMA) in air. A variety of nanometer composite oxides based on Y2 O3 that showed catalytic activity to many gas molecules were synthesized. For the fabrication of the detector, nanometer composite oxide was directly coated on the ceramic rod to form a 0. 1-0. 15 mm thick layer. The ceramic rod with nanometer composite oxide was inserted into a quartz tube with an inner diameter of 10 mm. The temperature of nanometer composite oxide was controlled by the digital heater. When gas samples passed through the nanometer composite oxide in the quartz tube by the air flow, the CTL was generated during the catalytic oxidation on the surface of the nanometer composite oxide. The CTL signals were respectively recorded by two ultra weak chemiluminescence analyzers. The CTL intensity and selectivity for the determination of benzene and TMA on nano- Zr3 Y2 O9 which was characterized by TEM were bigger and better than those on other nanosized composite oxides. The optimum experimental conditions were tested. Selective determination was achieved at a wavelength of 440 nm for benzene and 540 nm for TMA. The surface temperature of the nanometer materials was about 313 ℃. The flow rate of air carrier was about 140 mL/ min. The limit of detection of this method was 0. 30 mg / m3 for benzene at 440 nm and 0. 70 mg / m3 for TMA at 540 nm. The linear range of CTL intensity versus concentration of benzene at 440 nm was 0. 8-105. 0 mg / m3 , benzene at 540 nm was 3. 0-130. 0 mg / m3 , TMA at 440 nm was 2. 5-232. 0 mg / m3 and TMA at 540 nm was 1. 2-156. 0 mg / m3 . The recovery of 5 testing standard samples by this method was 96. 8% -102. 3% for benzene and 97. 6% -103. 4%for TMA. Common coexistence matters, such as formaldehyde, ethanol, acetone, ammonia, sulfur dioxide and carbon dioxide, did not disturb the determination. The relative standard deviation (RSD) of CTL signals of a continuous 200 h detection of gas mixture of 50 mg / m3 benzene and 50 mg / m3 TMA was 2. 0% , which demonstrated the longevity and steady performance of nano-Zr3 Y2 O9 to benzene and TMA under this experimental conditions.
